You will need your social security number and date of birth in order to complete your enrollment. This section contains detailed instructions and helpful hints for members or clients trying to log on to their web portal.

	Registration Process for First Time Visitors
	1.	Click on REGISTER
	2.	Select your USER TYPE  :  Employee or Dependent
	3.	Enter your GROUP NUMBER. This is either on your ID Card or was communicated during Open Enrollment.
	4.	Enter your Social Security Number.
	5.	You will get to select your own USERNAME and PASSWORD.
	6.	Complete the Registration Process.

Online Enrollment:
	If you are enrolling online, completing the Registration Process (above) leads to the Enrollment Tool and gives you access to the other Online Services.
	COMPANY CODE is the GROUP NUMBER from your ID Card
	Once the Enrollment is complete, you will receive a “Confirmation” page that you can print for your records.
When You Visit Again
	Enter your USERNAME and PASSWORD
	If you need to change your enrollment or information, select CHANGE MY ENROLLMENT. Please note: your employer must approve changes to your elections before they can be accepted.
	Forgot your USERNAME and/or PASSWORD? You can request a new one online if you have supplied an email address during the initial registration. Otherwise, please call Custom Design Benefits to get a new one at 513-598-2929 locally or toll-free at 1-800-598-2929 from 8:30 am to 5:00 pm EST.

	Registration for New Users
		To register, you need: the employee social security number, date of birth and email address
		Click on “New User”
		Complete the Registration Form
	When You Visit Again
		Enter your User Name and Password at the prompt.
