Your ID Card
Your Custom Design Benefits ID Card has contact information for you and your providers. Please be sure to use this each time you visit your physician or other healthcare provider. You can also use this card to get prescriptions at the pharmacy.
Some things to remember about the Custom Design Benefits Member ID Card:

Normally, we issue an ID card with the employee’s name only and the type of coverage they have elected. This helps simplify using the card for different members of your family. |

We typically issue one card per employee with single coverage and two per employee with family coverage. If you need additional cards, you can request them by logging on to your member self-service tool at MyPlan (login at left). |

Your Monthly Statements
Custom Design Benefits provides a monthly Explanation of Benefits for you and your dependents showing activity on your health plan during the prior month. If you have Express Scripts for your pharmacy benefits, the statement will also include prescriptions you have filled during that time as well.
This statement makes it easier to determine amounts you may owe for care you or a family member have received. If you need information sooner, please access your claims by logging on to MyPlan, the member self-service tool, on the left side of this page or contact us by phone.
