Your HIP Plan (Healthy Incentive Program)
HIP Plans (Healthy Incentive Programs) are employer-funded accounts used to supplement the primary carrier's plan.
HIP plans pay a portion of expenses directly to the healthcare provider. Each HIP Plan is unique to the employer who offers it.
When you receive services at a provider or pharmacy, present both the Primary Insurance card and the HIP ID card. This will allow the physician, facility or pharmacist to file a claim on your behalf. Most healthcare providers will submit claims for the patient to Custom Design Benefits. If not, use the information below to submit your claim to CDB.

No need to submit a claim if RX went through HealthSmart. If not:
Bag receipt*, Pharmacy print-out* or Primary Carrier EOB showing amount applied to Deductible or Out-of-Pocket. *Bag receipt or Pharmacy print-out should show the name of patient, drug name, cost.

Access to HIP Claims
There are several ways for members to access HIP claims:
- Call us at 800-598-2929 or 513-598-2929 (local Cincinnati area). We are available Monday through Friday, 8:00 am to 5:00 pm EST.
- Login to HealthVu. To login, members need the unique ID assigned by Custom Design Benefits on their ID Card and the password CDB mails to the employee's home address once they are enrolled. Members can change their password when they login for the first time. If you forget your password, you must call CDB to have it reset.
- Review the Monthly Statement sent to the employee's home address. Monthly Statements include all transactions processed (including RX) during the preceding month for all members covered by the employee.
