Your HRA (Health Reimbursement Arrangements)

Health Reimbursement Arrangements (HRAs) are employer-funded accounts used to supplement the primary healthcare insurance coverage offered.

Most of the time, HRAs reimburse a portion of the expenses that applied to the deductible under your medical plan. Each HRA is unique and specific to your employer's plan.

Submitting HRA Claims

  • Bag receipt*, Pharmacy print-out* or Primary Carrier EOB showing amount applied to Deductible or Out-of-Pocket. *Bag receipt or Pharmacy print-out should show the name of patient, drug name, cost.

    ONLINE Login to MyFlexOnline. Select Request Payment button at top of screen. Then email, fax or mail your receipts to Custom Design Benefits. Retain copies for your records.
    EMAIL Complete the HRA Claim Form and attach scanned receipts to your email. FLEXClaims@CustomDesignBenefits.com

    FAX Complete the HRA Claim Form and fax with receipts. No cover sheet is required.

    Checking Your HRA Balance

    There are several ways to view your HRA or check your balance:
    • Call us at 800-598-2929 or 513-598-2929 (Local Cincinnati area). We are available Monday-Friday, 8:00 am to 5:00 pm EST.

      For 24-hour access to balance information, call 866-598-2939 for automated phone service. (This service is only available if you login to MyFlexOnline for your HRA.)

    • Login to MyFlexOnline or, if CDB administers your Medical, Dental or Vision plan, login with MyPlan. Links at left. If you have trouble logging in, please call us.

    • Review your Explanation of Benefits. HRA Reimbursements include an Explanation of Benefits attached to the check showing any remaining balance.

    Getting HRA Reimbursements

    Custom Design Benefits will send you a check for the amount due which is then used to pay your healthcare expenses. We normally issue checks on a weekly basis.

    There are 2 ways to get HRA reimbursements from Custom Design Benefits:

    • Live Check sent to employee's home address

    • Direct Deposit of Reimbursements, if offered by your employer. You can sign up for Direct Deposit at any time. Please complete our Direct Deposit Form (located under FORMS & FAQs) and send or email it to Custom Design Benefits.

    Getting Online

    Registration for MyFlexOnline is easy. If assistance is needed, please call Custom Design Benefits to reset login or password. Unless you provided an email address during open enrollment or when you logged in for the first time, the online reset buttons will not work.
