Submitting FSA Claims
ONLINE Login to MyFlex. Select Request Payment button at top of screen. Follow prompts to complete claim form electronically. Then email, fax or mail your receipts to Custom Design Benefits. Retain copies of receipts for your records.
EMAIL   Complete the FSA Claim Form and attach scanned receipts to your email.  
FAX   Complete the FSA Claim Form and fax with receipts (no cover sheet required).

Accessing Your FSA
Members with our Flexible Spending Account (FSA) have several easy ways to access their account:

FSA CARD.  Use the take care FSA card – pay the healthcare or daycare provider with a quick swipe.
NEW! Certain expenses do not require any documentation! See more details about using your FSA Card here.
 
   
GET A CHECK. Pay the FSA expense directly, submit your receipt when you want to be reimbursed.  
   
DIRECT DEPOSIT. Use another form of payment, turn in the receipt and have the funds transferred directly into your bank account.
