Identifying chronic conditions early to keep members as healthy as possible for as long as possible is the goal of Custom Design Benefit’s Population Health Management team.
By helping members improve their health on an individual level, this approach also helps reduce the cost of health care for all.
Members who are found to have a 25% or higher probability of hospital admission or emergency room visits over the next twelve months make up the Population Health team’s case load. Once identified, targeted members receive an individualized care plan to help them avoid or reduce risks to their health.
From January 2020 to June 2023, CDB identified 1,487 members who could benefit from population health strategies and successfully engaged 79% of these members.
The Population Health Management team also addresses social determinants of health, recognizing that factors like food, transportation, and housing can profoundly impact health and connect members with community-based organizations to ensure services are in place.
