Submitting FSA Claims

WHAT WE NEED When using the FSA Card:
If the FSA Card is used, we will send a letter if anything is needed. Please do not send anything in unless you get a letter. Most transactions are tracked and do not need further documentation.

For anything other than FSA Card:
Depending on the type of FSA, information needed for claims will vary. Please see the FAQs page for more information on what is needed.

ONLINE If your employer uses Custom Flex:

Login to Custom Flex. Go to Home page. Click on "File a Claim" (top left hand corner). Select the reimbursement account in the "Pay From" box. Upload your documentation (must be JPG, JPEG, PNG, or PDF) then submit. Complete Claim Details. Submit. Retain copies for your records.
ONLINE If your employer uses MyFlexOnline:

Login to MyFlex. Select Request Payment button at top of screen. Follow prompts to complete claim form electronically. Then email, fax or mail your receipts to Custom Design Benefits. Retain copies of receipts for your records.
EMAIL Complete the FSA Claim Form and attach scanned receipts to your email. FLEXClaims@CustomDesignBenefits.com

Accessing Your FSA

Take Care Card

Members with our Flexible Spending Account (FSA) have several easy ways to access their account:

  • FSA CARD. Use your FSA card - pay the healthcare or daycare provider with a quick swipe.

    Certain expenses do not require any documentation while others now require a prescription from your doctor. See more details about using your FSA Card.

  • GET A CHECK. Pay the FSA expense directly, submit your receipt when you want to be reimbursed.

  • DIRECT DEPOSIT. By completing the Direct Deposit Form (available on FORMS & FAQs page), reimbursements can be deposited directly into your designated bank account when payments are made. This service is not offered by all employers.
